Back Exit Survey
You are invited to participate in a national survey intended to describe current clinical practice patterns and awareness of pediatric acute kidney injury incidence and outcomes in the pediatric intensive care unit. The study is being conducted by Dr. Amanda Hassinger at the Women and Children’s Hospital of Buffalo to better understand the divide between AKI research and what is clinically available and being used at the bedside.

The survey consists of 25 questions about acute kidney injury and should take no more than 10 minutes for you to complete. Your participation in this study is voluntary and you are free to withdraw your participation from this study at any time.

This study has been approved by the Children and Youth Institutional Review Board of the State University of New York at Buffalo. There are no risks associated with participation as the survey collects no identifying information of any respondent and all of the responses to the survey will be recorded anonymously. While you will not experience any direct benefits from participation, your involvement could inform future research and practical guidelines for the detection and management of pediatric acute kidney injury.

If you have any questions regarding the survey or this research project in general, please contact Dr. Amanda Hassinger at ahassinger@upa.chob.edu. If you have any questions concerning your rights as a research participant, please contact the Children and Youth IRB of the State University of New York at Buffalo, 716-878-7141.

By completing and submitting this survey, you are indicating your consent to participate in the study. Your participation is greatly appreciated.
