CSD Day on Capitol Hill Application

Hello:

Thank you for your interest in participating in the CSD's 2025 Day on Capitol Hill Event, taking place May 18-20. This event includes a welcome reception Sunday evening, May 18th, in-person training and social event/dinner on Monday, May 19th and meetings with legislators and a Congressional briefing on Tuesday, May 20th.

It's going to be a great time of being together with like-minded advocates, honing and using our advocacy skills to improve the lives of the 84 million Americans living with a skin condition.

The CSD is proud to offer our patient advocates financial support to aid their attendance by providing a travel bursary to help offset the costs of travel to the event. A two night stay, May 18/20, in a standard room at Yours Truly Hotel will be covered by the CSD. The CSD will also cover the following meals: reception snack and drinks on Sunday night, all meals on Monday, and breakfast and lunch on Tuesday. Any additional hotel nights, meals or incidentals will be the financial responsibility of the participant.

Each participant will be responsible to coordinate their own travel arrangements, which includes economy (no business or 1st class) flight or train and ground transportation, and parking. These expenses will be reimbursed to the regional amount specified, following the event upon submission of an expense report with accompanying receipts.

Any additional nights to the hotel stay due to travel needs must be approved by CSD or will be the financial responsibility of the participant.

Attendance to Hill Day training prior to the event and on May 19th, along with meetings with legislators on May 20th is mandatory for eligibility and reimbursement.

Applications will be reviewed by the CSD Advocacy Task Force and applicants will be notified by (X) of their acceptance.
