Hello Bernheim Forest community,
My name is Ashley Byrd, and I am an occupational therapy student at Spalding University. I am working with Bernheim Forest to collect data on nature-based play at Bernheim's Playcosystem. If you are a parent (18 years+) of a child who has visited Bernheim can you, please fill out a quick survey? This survey will give Bernheim and me data on children of all ages and abilities who play at the Playcosystem. Thank you for your time.

My name is Ashley Byrd. I am a third-year student in the Auerbach School of Occupational Therapy at Spalding University in Louisville, KY. You are invited to be in a research project about nature-based play and barriers to play at Bernheim Forest. This form will explain the details of the survey. Please read carefully. Feel free to contact us if you have questions about the study.

What are we trying to find out in this study?
The purpose of this study is to learn more about nature-based play and the barriers to providing a play environment for all children who visit Bernheim Forest’s Playcosystem. This study will inform the future development of Bernheim Forest. This survey will also provide Bernheim Forest with data to better understand the needs of the community.

Who can participate in this study?
You must be a parent (18+ years of age) of a child aged 0-21 years of age to complete this study.

Where will this study take place?
This study will occur at Bernheim Forest’s Playcosystem using a digital and/or written survey. 

What is the time commitment for participating in this study?
The survey should take 5-10 minutes to complete. 

What will you be asked to do if you choose to participate in this study?
You will be asked to answer a 6-question survey regarding your family.

What are the risks of participating in this study and how will these risks be minimized? 
There are minimal risks to you in this study and you will not be asked any personal information. 

What are the benefits of participating in this study?
This study will help to better understand barriers to nature-based play. 

Are there any costs associated with participating in this study?
This study is free.

Is there any compensation for participating in this study?

How will my data be protected?
Only the researchers will have access to your answers and informed consents. All digital information will be stored for three years in a password protected computer, and paper surveys and responses will be stored in a locked office available only to the researchers. Only summary data will be recorded.

What if you want to stop participating in this study?
Your participation in this research is optional. You can leave the study without penalty. Your participation and responses will not impact your membership or access to Bernheim Forest in any way. This consent document has been approved for use for one year by the Research Ethics Committee [REC]
