Southern Illinois Healthcare is conducting an assessment of health needs in the communities we serve and we would appreciate your participation. The survey results will help us identify the most pressing health concerns in our area to best meet community needs.
The confidential survey is being administered by Crowe LLP on behalf of SIH. The results will be analyzed, along with other data, to help develop a comprehensive health needs assessment to drive our work to improve community health.
